The Health Information Management Department at Grand View Health protects the confidentiality of your medical records while allowing your healthcare providers to deliver consistent quality care. To obtain a copy of your medical records, refer to the information below.

To request your medical records, complete the Authorization for Release of Health Information (Patient Authorization for Release form), available for download here. You can also request the form at any of our outpatient centers. Once you’ve completed the Patient Authorization for Release form, there are four ways you can send it to us:

People Who are Authorized to Sign for the Release of Your Health Information:
Yourself (Not your Spouse)
A parent if the patient is under 18 years of age
A minor, age 14-17 years, receiving psychiatric, drug/alcohol services, or who has a child
Both parents and minor if the patient is age 12-17 years and receiving drug/alcohol services
Legal Guardian (must provide proof of guardianship)
Power of Attorney if patient is unable to sign. (the Power of Attorney documentation must be presented)
Executor/representative of estate for deceased patient and a copy of a “short form” death certificate presented. (must provide proof of Executorship)
Completely fill out the form in its entirety. Include dates of service (if no dates are given, we will give the last 2 years of information) and type of information needed, for example:
History & Physical
Final Summary
Consultation
Emergency Room Report
Operative Report
Pathology Report
EKG
Laboratory Reports
X-Ray reports/films/imaging reports

If you request records other than for continued care, an invoice will be issued for your payment. The invoice will be sent to the address listed on the Patient Authorization for Release form. Grand View Health will mail the records to you or records can be picked up at Health Information Management at Grand View Health. Payments can be made by Credit Card or Check. Please make all checks payable to “CIOX Health”. Fees for medical records are state regulated and HIPAA compliant for patient requests.
Medical records are mailed, not faxed, about 7-10 business days from the date that the signed request is received. If you need your records immediately or want to pick them up in person, call 215-453-4850. Photo identification is required if you are picking up your medical records in person.
If you wish to have your records sent to another healthcare facility other than the ordering facility or a Grand View affiliate, you will need to complete a Authorization for Release of Health Information (Patient Authorization for Release form), available for download here, with the receiving healthcare facilities information on it as the disclosed party.

Grand View’s Patient Portal – MyGVH.org – is an online tool that allows patients to access their healthcare information easily and securely. MyGVH.org will allow patients to electronically manage their own health record as well as their dependents health records.
MyGVH.org will provide patients with:
Health Information: Lab Results, Visit History, Discharge Instructions and a Health Summary
Allergies and Medications: Allergies and Conditions, Immunizations and Medications with Instructions
Appointments: Appointment Requests as well as Upcoming Visits
Billing (Coming Summer of 2014): Outstanding balances and Historical Charges and Statements
Complete and sign the Patient Portal Enrollment Form ( you can find it here). Hard copies available at most GVH Locations.
Bring your completed form to Patient Registration or the Health Information Management Office to get signed up. ID required to complete enrollment. Enrollment process will be completed no later than 3 business days.
Within 3 business days you will receive an email with your one-time user name, password, and link to the Portal.
Click on the link to access the Portal.
Enter your one-time user name, password, and security question and click Log On.
Enter your new user name and password.

The patient needs to bring the completed form to Patient Registration or the Health Information Management Office to get family members signed up.
Access your email for your one-time user name, password, and link to the Portal.
Click on the link to access the Portal.
Enter your one-time user name, password, and security question and click Log On.
Enter your new user name and password.

MyGVH.org allows you to give access to others to view some or all of your health record. You have control over what information can be viewed, choosing between billing, family history, profile, clinical data, or a combination of all categories. This access can be removed at any time once an invite has been sent.
To give access:
Log on to the portal, click the Profile Button and then the Shared Access button to create an email invitation and select the areas that can be viewed.
If the invitee is a MyGVH.org portal user, please confirm and use that person’s portal email address before sending the invitation.
To accept access:
If you are a MyGVH.org portal user, click the Accept Patient Portal link in the email invitation and log on to the MyGVH.org patient portal.
If you do not yet have a MyGVH.org Account, click on the Accept Patient Portal link in the email invitation and then the Accept Your Invitation link on the logon page; enroll in the portal using the same email address as your invitation.
The patient portal can be accessed through the following Apps, the Grand View Health App and the MHealth App. All apps are available in your App store and Google play Store. My GVH.org Patient Portal personal health information can also be accessed through the Health Records App on your IPhone. The App is available on your Iphone device.
