Diabetes Self-Management Training
An Interactive Approach to Diabetes Management
Grand View Health Diabetes Self-Management Training is a series of interactive classes led by our registered dietitians and certified diabetes educators, providing comprehensive information about diabetes management. Recognized by the American Diabetes Association (ADA), this program is designed for anyone with diabetes or pre-diabetes, whether you are newly diagnosed or have had the condition long-term.
We will review topics such as meal planning, medications, how to prevent complications, exercise and lifestyle adjustments. You will participate in a one-on-one introductory session, as well as four two-hour small group sessions featuring ADA’s Conversation Map Program.
Conversation Maps are patient-centered, innovative tools for diabetes management education that encourage interaction and collaboration among patients, materials and educators in a friendly, open environment. They allow patients to discover solutions on their own and make positive lifestyle changes. Topics include:
