Assessing the Health Needs of Our Community

Health is influenced by many factors, including social and economic conditions, the built environment, accessibility of health products, the behavioral choices people make and access to and quality of the medical care system. Hospitals play a unique role addressing many of these factors both in providing medical care and investing in initiatives to improve the health and well-being of communities they serve.

To identity and prioritize the health needs of the community it serves, and to meet requirements of the Affordable Care Act (ACA), Grand View Health joined a first-ever Southeastern PA (SEPA) Regional Community Health Needs Assessment (CHNA), with a specific focus on Bucks, Chester, Montgomery and Philadelphia counties. The collaborative, community-engaged approach involved collecting and analyzing quantitative and qualitative data from a variety of sources to comprehensively assess the health status of the region.

With this local and regional perspective of health needs, Grand View Health developed an implementation plan to address those needs most relevant in its service area.
