To register for your surgery or procedure, Grand View Health uses One Medical Passport, an online medical history questionnaire that allows you to enter your health information anytime from anywhere. Please complete this information as soon as your surgery or procedure has been scheduled.
Completing a One Medical Passport is easy
For most patients, it takes less than 30 minutes. Be assured that One Medical Passport uses the highest grade Internet security so all of your information is kept confidential and is never released to anyone other than your medical team. Your information will be saved, so you can quit filling out the questionnaire and return later to complete it. You can also print out a copy of your medical history after you create it online and keep it with you or with your other healthcare documents.
Items you’ll need to complete One Medical Passport
- Your health insurance information
- The names, addresses and phone numbers of your physicians
- A list of all medications you are taking and dosages
- A list of all surgical procedures you have had
- The name of the surgeon who will be performing your upcoming procedure
After your medical history is complete, one of our preoperative review clinicians will be able to access the information you entered online. This information will assist the nurse in organizing and documenting your complete medical history to prepare for your surgery. As part of this process, you may receive a telephone call from a member of our preoperative review team to review your history online with One Medical Passport.
