Register for St. Luke’s Physical Therapy Portrait

Grand View Health Physical Therapists will need to have their official photo taken at one of the sessions listed below by the St. Luke’s marketing department.

Due to specific lighting requirements for St. Luke’s portraits, all photos will be taken at the hospital in the ICU waiting room. Please register for a time on the form below. Photo sessions are 5 minutes and there is a restroom available.

Location: Grand View Hospital, West (legacy) Building

Directions (follow signs to “Chapel” or follow the directions below):

• Enter campus and follow signs to Employee Parking (the lots behind the old Emergency Department)
• Use Employee Entrance
• Once inside doors, take elevator to your immediate right
• Go to Floor 2
• Exit elevator to your right

Wardrobe:

• Staff portrait requests may be made for the following reasons:
  • Photo Business Cards
  • Department Rosters for Website
• What to wear: please no t-shirts or scrubs (unless related to a special marketing project)
  • Men – collard shirt, tie, and jacket required
  • Women – nice blouse or equally professional attire
  • We honor the Employee Handbook code which states: Visible face or body jewelry are not permitted (i.e., nose, lip, eyebrow or tongue jewelry are not permitted). No employee shall be permitted to wear more than two traditional earrings in / on each ear.
• Portraits are kept on file in Media Production Services and are available upon request
