Grand View Health working collaboratively with local primary care offices announces the formation of an Integrated Delivery Network – Grand View Healthcare Partnership, LLC (GVHP). An Integrated Delivery Network is a group of doctors, hospitals, and other health care providers who unite to provide coordinated, high quality care. The goal of coordinated care is to ensure that patients, especially the chronically ill, get the right care at the right time, while avoiding unnecessary duplication of services.
Founding members of GVHP include Grand View Health and its owned Grand View Medical Practices; TriValley Primary Care, PC; Kim Kuhar, DO, Internal Medicine, PC; County Line Medical Associates, PC; Harleysville Medical Associates, PC; and Steven Sklar, DO, PC. GVHP members represent nearly 50 primary care physicians and mid-level practitioners, along with a host of specialists, located throughout Bucks and Montgomery Counties.
According to Keith Hammerschmidt, director of GVHP, the organization’s mission is “to promote the wellness of our community through the delivery of coordinated healthcare with a focus on quality, patient experience and cost effectiveness.” With the collaboration of its primary care providers and specialists, GVHP will closely manage the healthcare needs of patients based upon evidence-based medicine with a focus on improving the health of the population, enhancing the experience and outcomes of the patient, while reducing the cost of care.
