Family Medicine – Various scheduling options available

Conducts patient evaluations, including screening patients to determine need for physician attention, taking patient history, performing physical examinations, reviewing records to determine health status, making decisions regarding appropriate management and treatment for the initial evaluation of a problem.  Responsible for collecting specimens, prescribing treatments and completing referrals.  Previous experience working in family practice preferred.

Palliative Care – Various scheduling options available

The CRNP will be functioning as a part of the Palliaitve care team, is directly responsible to the Palliaitve Care Manager and will receive clinical guidance from the Palliative Care Medical Director.  Will provide a full range of health care services including planning, organizing, coordinating and directing optimal patient care and outcome to assure quality patient care to the pediatric, adult and geriatric patient that meets regulatory requirements.  Candidate will monitor patient response to palliative plan and adjusts plan accordingly. Must have excellent inter-personal and communication skills.  Experience in symptom and pain management required.

Must be a licensed Nurse Practitioner in Pennsylvania and possess a Controlled Substance license in PA. A minimum of 2 years of RN or NP experience in a Home Care setting preferred. Current PA Driver’s license and automobile insurance required. Daily working conditions may involve sustained physical effort such as lifting up to 25lbs or moving up to 150lbs such as in moving patients.  Computer eye strain is possible. Stress due to time pressures. Travel within hospital service area is required, at times during inclement weather.
