Instructions for Using this Card
- Complete the card by filling in the requested information.
- Carry your medication card with you in any convenient location – like in your wallet or purse.
- Show the card to your doctor, pharmacist or health-care professional when asked about medications.
- Have your doctor review your medication card when you have a visit. Keep it up-to-date.
- Once a year, place all of your prescriptions, vitamins and herbal supplements in a brown bag and take them to your doctor to review.
