When medically appropriate, your doctor will write an order allowing you to leave the hospital, or to be “discharged.” The discharge order specifies the date and time you may leave Grand View, as well as any special instructions about needed care after you leave. Once you receive your instructions, we suggest that you arrange for someone to take you home, as it is not advisable to drive immediately after hospitalization.


Planning Ahead for Your Discharge
We recognize that discharge from the hospital can be a very hectic time for most patients. To help you prepare for your safe return home, a Grand View Case Manager may contact you.Your Case Manager can assist you in nursing home placement as well as arrange for speech and occupational therapy, physical therapy, home health nurses, homemakers and home health-care equipment. The Case Manager may also help you and your family cope with any emotional, social, physical and financial concerns related to your illness.

Home Care
When you need medical or supportive services in your home, the specially trained professionals of Grand View Hospital Home Care can assist you. The Home Care staff serves residents of Upper Bucks and Upper Montgomery counties. For more information, call 215-453-4265.

Hospice Care
Grand View Hospice, a service for the terminally ill, focuses on relieving symptoms of disease and providing educational, emotional and spiritual support. As in Home Care, specially trained professionals visit the patient at home to teach both patient and caregivers how to manage symptoms and to assess changes in the patient’s condition. Emotional and spiritual support is offered, and a registered nurse is on call 24 hours a day for emergencies. Hospice care is covered by most insurance plans. Health-Care Equipment/Supplies
Grand View’s Medical Equipment Department offers a full range of medical equipment, supplies and services – such as beds, wheelchairs, oxygen and walkers – to assist patients with their specific needs after leaving the hospital. If you have questions or need information on specific items or services, call extension 4700.

Ambulance Transport Service
Grand View Medical Company offers an ambulance transport service for non-emergency situations, such as patient admissions and discharges. Most insurance plans consider this service for payment. If you have questions or need more information, call 215-257-8661. Information on other ambulance transport services may be obtained from the Case Management Department at extension 4487.

Support Groups
Grand View Hospital offers a variety of support groups to patients and their families. These groups provide education and support in a sharing, understanding environment. For more information on these programs, refer to the list in your packet or call the Grand View Health Line at 215-453-9676.
