Coordinating Your Care While You're in the Hospital and Arranging a Safe and Supportive Discharge
There is usually no better place to be than a hospital when you are seriously ill or injured. However, most people can't wait until their health improves so they can go home. At Grand View, our nurses, doctors, social workers and other staff work together to speed your recovery. Coordinating their efforts is the case manager, who "works behind the scenes" to make sure everything goes according to plan.
