Grand View Hospice, a trusted leader in palliative end of life care since 1981, offers personalized services to terminally ill patients and families in their homes, personal care, assisted living, and nursing home environment.We recognize your need for compassion and support at this critical time in your lives. Grand View Hospice uses an Interdisciplinary Team approach, providing care that encompasses the physical, emotional, spiritual and social needs of everyone involved in your care.
Referral
Hospice care is available to terminally ill patients who are no longer under treatment for cure and have a limited life expectancy. The patient’s primary physician usually initiates the referral process, although case managers, hospital or nursing home personnel, clergy members, family members, or other health care professionals may also refer patients. Our experienced Admissions Team will arrange follow-up care. Hospice provides services to patients and their families, regardless of race, ancestry, color, religion, gender, age, disability, sexual orientation or national origin. Our staff provides assistance to those who do not speak English or are hearing impaired.
Services
Grand View Hospice provides an Interdisciplinary Team approach to coordinate patient and family services including:
- Physician-directed symptom control and pain management
- Skilled nursing visits in your residence with support as needed from physical, occupational, speech, respiratory therapists and dietitians
- Social worker visits addressing the emotional and psychosocial needs of patients and their families
- Admissions team providing prompt and consistent referral follow up
- Visiting home health aide delivering personal hands-on care
- Emergency on-call services, 7 days a week, 24 hours a day
- Volunteers offering support and companionship
- Chaplain services giving spiritual support to patients and their families
- Various complementary therapies available
- Bereavement counselor/bereavement program supporting family members following the loss of a loved one through grief educational mailings, individualized support and support groups for adults and children
Payment for Services
Many health insurance programs, such as Blue Cross, Medicare, Medical Assistance and other insurers, provide coverage for hospice care. Fees for services not covered by these traditional insurance programs are established based upon the patient’s ability to pay. As is the policy for all Grand View Hospital services, patients in need of services are never denied care due to an inability to pay.
Grand View Hospice is Medicare certified, Pennsylvania State Licensed, accredited by the Joint Commission on Accreditation of Healthcare Organizations, and a member of the Pennsylvania Hospice Network.
You Can Help
The personalized care provided by Grand View Hospice depends greatly on a special group of people—our volunteers.
Under the supervision of the Hospice Volunteer Coordinator, volunteers are trained by the professional staff of Hospice and Grand View Hospital. This comprehensive training program provides volunteers with the skills they need to fulfill their volunteer role. Training topics include: the history and philosophy of the hospice concept, communication skills, loss and grief issues, spirituality and bereavement. Volunteers may serve in the following areas: patient care, buddy visits, 11th-hour support, bereavement, office and courier.
Grand View Hospice welcomes your generosity and financial assistance as a means of ensuring the continuation of hospice services for our community. Memorial donations, bequests, community organization support and all types of charitable contributions designated for hospice care should be directed to the Grand View Health Foundation, Hospice Program.
