Qualifications
Grand View Hospital's Home Care Department is a Medicare-certified home health agency. It is also accredited by the Joint Commission on Accreditation of Healthcare Organizations.
Services Available
- Part-time intermittent skilled nursing, physical therapy, occupational therapy, speech therapy, medical social work services and home health aide services.
- Specialty services include home intravenous therapy and maternal/newborn visits.
Criteria for Services
- Physician's order for service.
- Patient lives in Central, Upper Bucks and Montgomery Counties.
- Patient may request Grand View Home Care services on discharge from a hospital or through his/her physician.
- Patients can use Grand View Hospital's Home Care services even if they receive treatment at another hospital.
Insurance Coverage
Grand View Home Care accepts Medicare, Medicaid, Independence Blue Cross, Aetna and many other insurance plans. Coverage for a person's particular needs are discussed on admission to service.
