Why Have a PRIMARY CARE PHYSICIAN?
When it comes to your health, one of the most important things you can do is to choose a primary care physician. PCP’s focus on caring for the patient as a whole. They’re knowledgeable about a broad range of general medicine and often have extra training in a specialty such as family practice, internal medicine or pediatrics. Benefits of having a PCP include:

A Good Starting Point
Your PCP will either be able to treat your illness or serve as a liaison to specialty care providers.

With routine checkups and screenings, your PCP can help in early detection of more serious conditions.

The Adult Hospitalist Program: Communicating Details of Your Hospital Stay to Your Health Care Provider

The health care system is undeniably complex. If you’re a patient in the hospital, who communicates the details of your stay to your health-care provider, like your family practitioner or nursing home? Someone must assume the responsibility.

Grand View Hospital introduced its Adult Hospitalist Program in July 2004. Hospitalists are physicians whose primary focus is the general medical care of hospitalized patients. A doctor from the hospitalist group is available at Grand View 24 hours a day, 7 days a week to meet patient care needs.

The goal of the Adult Hospitalist Program is to improve communication between GVH and community health-care providers. We want to ensure the events of your hospital stay are directly and accurately communicated so you receive proper follow-up care.

Most patients are referred to a hospitalist by their primary care physician (PCP) for treatment during their stay and then returned to their PCP after discharge. In general, patients who do not fall under the care of a specialist are cared for by a hospitalist – whether they have a PCP or no physician at all.

In fact, nearly all of the country’s leading hospitals embrace hospital medicine and look to hospitalists to improve patient satisfaction, reduce patient care costs and provide additional professional resources for quality and patient safety initiatives.
