Workforce Influenza Immunization Program
The flu is a contagious respiratory illness caused by influenza viruses that infect the nose, throat, and lungs. It can cause mild to severe illness, and at times can lead to death. The best way to prevent the flu is by getting a flu vaccine each year. All hospital employees and other workforce members (including physicians, patient care contract workers and volunteers) are required to receive the seasonal influenza vaccine by October 31, 2013. Exemption requests in completion must be received by September 15th. The Workforce Influenza Policy and Exemptions Forms have been updated for the 2013-2014 flu season.

No need to get another one! If you received the 2013-2014 seasonal influenza vaccine elsewhere, provide proof of immunization to Employee Health.Acceptable proof of immunization includes:
-A physician's note
-A copy of the immunization record
Documentation may be scanned and email to AskHR@gvh.org, faxed to 215-453-4030, mailed to the HR Department or dropped off to the HR Department.
