Grand View Hospice staff recognizes the need for compassion and support at this critical time in someone's life. Grand View Hospice staff uses an interdisciplinary team approach, providing end-of-life care that encompasses the physical, emotional, spiritual and social needs of everyone involved.
Hospice care is available to terminally ill patients who are no longer under treatment for a cure and have a limited life expectancy. The patient's primary physician usually initiates the referral process, although case managers, hospital or nursing home personnel, clergy members, family members or other health-care professionals may also refer patients.
Grand View's experienced admissions team will arrange follow-up care.
Grand View Hospice staff provides a variety of
services to patients and their families, regardless of race, ancestry, color, religion, gender, age, disability, sexual orientation or national origin. The Grand View Hospice staff also provides assistance to those who do not speak English or are hearing- or vision-impaired.
