Transitions in Care Program - Palliative Care
The Palliative Care Program at Grand View Hospital provides comfort care and symptom management for seriously ill patients while they continue to receive curative treatment. The program also assists you and your family members in understanding the choices available for your care.

The Grand View Medical Equipment Company staff offers a full range of home health-care equipment and supplies, from hospital beds to breast pumps to home oxygen systems. Our knowledgeable, courteous staff delivers items to your home and is available 24 hours a day, seven days a week for emergency service.
Maternal-Newborn Home Care
Maternal-Newborn Home Care services are available to all maternity patients including those who have delivered at facilities other than Grand View Hospital.
Case Management
Should you need to visit the hospital, the nurses and social workers of Grand View's Case Management Department will coordinate a smooth transition from hospital to home, hospice or long-term living facility.
