Community Nurse - Home Care
The Community Nurse - Home Care Department at
Grand View Hospital provides Grand View's quality nursing and other health care services in the convenience and comfort of your own home. A Medicare-certified home health agency, our home care program is also accredited by the Joint Commission.
Home care services provided include part-time intermittent skilled nursing, rehabilitative therapy (physical therapy, occupational therapy, speech therapy), medical social work services, and home health aide services. Specialty home care services including home intravenous therapy and maternal/newborn visits.
Skilled Nursing
- Dressing (bandage) changes and wound care
- Catheter care
- Diabetes care and education
- Injections
- Intravenous therapy
- Medication monitoring
- Pain management
- Patient and family education
- Patient assessment and monitoring
Rehabilitative Therapy
- Physical therapy to help with safety, balance, strength, range of motion, pain and mobility.
- Occupational therapy to help with activities of daily living like dressing, grooming, bathing and preparing meals.
- Speech therapy to help improve speech, language and cognitive function as well as swallowing problems (dysphagia)
Medical Social Work
- Counseling and education
- Coordination of and referral to community resources
- Short-term counseling to assist with adjustment to illness and use of support services
Home Health Aide Services
- Bathing and personal hygiene
- Dressing
- Meal preparation
Criteria for Home Care Services
- Physician's order for service
- Patient lives in Central, Upper Bucks or Northeastern Montgomery County
- Patient may request Grand View Hospital home care services on discharge from a hospital or through his or her physician
- Patients can use Grand View home care services even if they receive treatment at another hospital
Home Care Insurance Coverage
The Grand View Hospital Community Nurse - Home Care Department accepts Medicare, Medicaid, Independence Blue Cross, Aetna, and many other insurance plans. Coverage for a person's particular needs is discussed on admission to service.
